2. What is TLD?
A thermo luminescent dosimeter, or TLD, is a type of radiation
dosimeter. A TLD measures ionizing radiation exposure by
measuring the intensity of visible light emitted from a crystal in
the detector when the crystal is heated. The intensity of light
emitted is dependent upon the radiation exposure.

3. Why its Important?
 Personal dosimeters (also known as TLD's) are used to assess
ionizing radiation doses to individuals.
 They are worn by persons working with ionizing radiation to
assess their cumulative radiation dose over periods of up to
three months.

5. TLD Management
 TLD Card should be loaded in cassette before use.
 Always wear TLD While working in Radiation Field.
 Designated Storage for away from Radiation Facilities.
 Keep the Control TLD at designated Place.
 Don’t Share TLD with Other’s.
 Quarterly Monitoring of Radiation Dose
 Maintain a separate register for TLD arrival and dispatch for each quarter

6. How to Apply for TLD Badge
 There are two forms:
a) PMS -1 Form (Personnel Monitoring Service Form) – For Registration of
Institutes.
b) PDF-2 Form (Personal Data Form) – For Registration of Radiation
Workers.
 New Institute must send both the forms

7. Personal Monitoring Services Form-1
(PMS – 1)
 One Common form i.e PMS-1form for new Institution
should be filled with complete detail of Institute and
their employee.
 PMS-1 needs to fill only once till the time center exist .
 This form is being filled to get center/institution registered under Ultratech.
 TLD of staffs are issued under the institution ID only.

8. Personal Data Form (PDF-2)
 PDF -1 Forms will be separately filled for all employees whose TLD
will be required (Radiologist, Technician, Nursing etc).

9.  Only single or one PMS-1 Form is required for whole institute and One
PDF Form is to be filled for each person/worker.
 PMS Form does NOT require any document. A covering letter on PRINTED
LETTER HEAD of the Institute should be attached.
 PDF form requires a recent color photograph of exact size (mentioned on
the form) and copy of any Govt. Approved Photo ID.
 It is mandatory to mention e-LORA Institution Registration Number in the
form and Institution Number if the Institute is already registered.
 Forms should be completed by affixing Rubber Stamp & Signatures on
required places.
 Two copies of each form i.e. one original copy & one photocopy of the
original is to be sent preferably by Speed Post to our address. It is
advisable to keep one photocopy of each form in your records

10.  With Application form Advance payment should be done.
 Soft copies of these forms, Payment Receipt, Courier Receipt need to be
sent by email to utechlab@rediffmail.com if there is any difficulty in filling
up these forms
 Alternatively the forms can be filled from Utech Lab web site and after
receipt of approval from Lab the printout can be taken, copied and sent
by SPEED POST.
 After receipt, all the correct & complete forms are forwarded to the BARC
by the Lab for further processing and allotment of Institution ID Number
& Personnel Numbers.
 After receipt of clearance from the BARC, the TLD Cards are dispatched to
the Institutes.

11. TLD Charges
 Registration Charge: Rs. 150/- per TLD Card/Badge +GST 18%=177
 Service Charge: Rs. 190/- per TLD +18% GST Card/Badge for each Service
Period-(224)
 Holder/Cassette (in case of lost or broken holder) is Rs. 83/- per Holder.
 Charges for Non-return of Cards: Rs 250/- per Card +GST 18% (TLD Cards
MUST be returned by end of service period).-295
 5. Delayed Payment: will bear interest charges at 15% per annum basis, plus
penalty (if warranted).
 Note: These annual charges are to be paid in ADVANCE at the beginning of
service and subsequently in the month of April each year. All the Additions
OR Deletions are adjusted in FINAL BILL at the end of financial year (April-
March).
