19. The Gate Control Model. Large-diameter afferent fibers (L) stimulate the substantia gelatinosa (SG)
and the transmission cells (T) in lamina 4. The SG cells reduce the membrane potential of afferent
terminals, thus producing presynaptic inhibition. Small-diameter afferent fibers (S) also stimulate the
transmission cells but inhibit SG cells and thus turn off the existing presynaptic inhibition. Published

21. Schematic representation of the generation of
pain: (A) Normal: Central terminals of c-afferents
project into the dorsal horn and make contact with
secondary pain-signaling neurons.
Mechanoreceptive Aβ afferents project without
synaptic transmission into the dorsal columns (not
shown) and also contact secondary afferent dorsal
horn neurons. (B) C-fiber sensitization:
Spontaneous activity in peripheral nociceptors
(peripheral sensitization, black stars) induces
changes in the central sensory processing, leading
to spinal-cord hyperexcitability (central
sensitization, gray star) that causes input from
mechanoreceptive Aβ (light touch) and Aδ fibers
(punctuate stimuli) to be perceived as pain
(allodynia). (C) C-fiber loss: C-nociceptor
degeneration and novel synaptic contacts of Aβ
fibers with “free” central nociceptive neurons,
causing dynamic mechanical allodynia. (D) Central
disinhibition: Selective damage of cold-sensitive
Aδ fibers that leads to central disinhibition,
resulting in cold hyperalgesia. Sympat,
sympathetic nerve .

A simplified representation of the central and peripheral components of the stress system, their functional interrelations and their relationships to other central nervous system pathways involved in the stress response. CRH: corticotropin-releasing hormone, LC/NE sympathetic system: locus ceruleus/norepinephrine-sympathetic system, POMC: proopiomelanocortin, AVP: arginine vasopressin, GABA: γ-aminobutyric acid, BZD: benzodiazepine, ACTH: corticotrophin, NPY: neuropeptide Y, SP: substance P. Activation is represented by solid green lines and inhibition by dashed red lines.
