The Health Care System For The 21st Century
Introduction By 2001 it was brutally apparent that the U.S. Health Care system was in dire need of a
reform in regards to quality and patient safety. Following two separate reports issued by The
Institute of Medicine (IOM), To Err is Human (1999) and Crossing the Quality Chasm: A New
Health Care System for the 21st Century(2001) the U.S. Congress requested the IOM review quality
processes across multiple government funded health care programs. And understandably, "these
reports described America's healthcare system as a tangled, highly fragmented web that often wastes
resources by duplicating efforts, leaving unaccountable gaps in coverage, and failing to build on the
strengths of all health professionals" (Brown J., p. I – 15, 2013). Thus, the Committee on the Quality
of Health Care in America released 6 aims to address key dimensions that require improvement in
our health care system. These aims propose that our system needs to strive to be more Safe,
Effective, Patient–Centered, Timely, Efficient, and Equitable (STEEEP). All of which were created
to help overhaul our current health care system and, more importantly, narrow the quality chasm.
Safe
Crossing the Quality Chasm defines safe as, "avoiding injuries to patients from the care that is
intended to help them" (p. 5). Ideally, this aim is to help protect patients from harm, improve on safe
patient–care practices, and utilize the most up to date evidence–based medicine for better outcomes.

Many individuals confuse a core idea concept with a positioning concept. A Core Idea Concept
simply describes the product or service. Its purpose is merely to determine whether the idea has any
interest to the end buyer. In contrast, a Positioning Concept attempts to sell the benefits of the
product or service to a potential buyer. The positioning concepts focus on the rational or emotional
benefits that buyer will receive or feel by using the product/service. A successful positioning

The Roots Of Patient Safety Essay
The roots of patient safety may be traced back to the 19th century. Florence Nightingale an English
nurse linked poor living conditions with the high death rates of soldiers. Ignaz Semmelwies an
obstetrician noticed the importance of hand washing in medical care. Patient safety in the United
States came to the forefront in the 1990s and since has developed into a new health care discipline
focusing on preventing adverse health care events.
The Institute of Medicine (IOM) and other academic research paved the way in brining light to the
issue of patient safety in the U.S. One of the first pushes for patient safety was with the introduction
of the Electronic Health Record (EHR) then called the Computerized Patient Record (CPR) in a
report The Computer–Based Patient Record: An Essential Technology for Health Care published in
1991 by IOM. The report outlines the "quality, safety, and efficiency" the EHR will bring to
physicians.
One of the most significant IOM reports revealing the importance of patient safety came in 1999
with the publication of To Err is Human: Building a Safer Health System by the Committee on
Quality of Health Care in America. The astounding findings of this report propelled patient safety to

Hand hygiene (HH), or cleansing of the hands with soap and water or an alcohol–based hand rub
(ABHR), is the single most important clinical determinant for marked risk reduction of hospital–
acquired infections (HAIs) or more specifically, multidrug–resistant organisms (MDROs), a major
contributing factor to patient morbidity and mortality. Despite being one of multiple quality
indicators for hospital patient safety ratings and infection prevention interventions, hand hygiene
compliance (HHC) amongst healthcare workers (HCW) is subpar across the nation. This paper
explores the significance of HHC, a current quality improvement issue in nursing practice. Current
data and tools for measuring the nursing sensitive indicator of HHC will be discussed. It concludes
with a model for improvement in HHC to meet a minimum recommended sustained rate of 90%
compliance.

Institute of Medicine (IOM) has had a profound impact on health care in America and the view of

different times between 1996 and 1998 to look at changes regarding quality in health care. Within

experience? What would it look like? How would they benefit? I plan to discuss these hypotheticals
through research regarding Patient–Centered Care. After all, studies and surveys performed by the
Institute of Medicine and the Institute for Healthcare Improvement, read that engaged patients in
any health care setting, have better perceived health outcomes, thus higher actual quality patient
outcomes.
Relating to the above, I believe that engaged learners have a better understanding of the material and
thus more effective practice outcomes. Because of this belief, I would use informal lecture coupled
with a series of patient narratives and class discussions regarding the patient–centeredness of care–
team and patient interactions. In teams, we would discuss patient outcomes, care team involvement
and behaviors, provider prompts, patient prompts, professional responsibility, what was and was not
patient–centered, barriers to care and areas of opportunity to meet patient needs. After discussion,
those learning would be responsible to research an assigned condition and to create a patient
centered experience for either a primary care or acute interaction.

other characters in the book. As the development of Svidrigailov's character furthers into the book, it
reveals more and more of his dangerous personality. Svidrigailov begins to disclose his underlying
motives and theory that he is a strong man who can be alone by himself in order to survive.
Svidrigailov's foundation of his mental stability and his arrogant mindset is from his expertise to
control people and the situations to fulfill his desire for power and self–enjoyment. This can be
supported by his abusive nature towards women and that through physical violence, it proves that he
has the power over people. However as Svidrigailov begin to lose his control over people especially
the women in his life, his fundamental beliefs begins to be shaken, altering his personal view of
himself that he is no longer a strong, individualistic, and an isolated man. Svidrigailov's loss of
power is validated when Dunya rejects his marriage proposal. Dunya's strong will and individualism
combats head–on with Svidrigailov's creating a battle between the strength of minds. Eventually,
Svidrigailov loses this battle and, mentioned in my journal, "realizes that he can't be alone in society
and needs a companion in his life." As Svidrigailov enters into isolationism, he also embarks the
journey of long–suffering thus proving once again the danger of isolationism. With everything gone
from his life, Svidrigailov decides to end his life. Even with his death, Svidrigailov shows his
individualism and domineering nature by controlling his own death. Back then people think that
suicide is sinful because one has no right to act as the Will of God thus by doing so, it actually
separates one from God entirely.

The Health Care System For The 21st Century Introduction By 2001 it was brutally apparent that the U.S. Health Care system was in dire need of a reform in regards to quality and patient safety. Following two separate reports issued by The Institute of Medicine (IOM), To Err is Human (1999) and Crossing the Quality Chasm: A New Health Care System for the 21st Century(2001) the U.S. Congress requested the IOM review quality processes across multiple government funded health care programs. And understandably, "these reports described America's healthcare system as a tangled, highly fragmented web that often wastes resources by duplicating efforts, leaving unaccountable gaps in coverage, and failing to build on the strengths of all health professionals" (Brown J., p. I – 15, 2013). Thus, the Committee on the Quality of Health Care in America released 6 aims to address key dimensions that require improvement in our health care system. These aims propose that our system needs to strive to be more Safe, Effective, Patient–Centered, Timely, Efficient, and Equitable (STEEEP). All of which were created to help overhaul our current health care system and, more importantly, narrow the quality chasm. Safe Crossing the Quality Chasm defines safe as, "avoiding injuries to patients from the care that is intended to help them" (p. 5).
