- It is commonly accepted that we can only achieve this by means of delivering a new type of healthcare model – what has become commonly known in Ireland as Connected Health. Now, before we go any further, I think it is useful to consider what we mean when we talk about Connected Health.The Partner’s Healthcare Centre for Connected Health in Boston defines CH as a model of healthcare delivery that uses technology to provide healthcare remotely.In fact, I think that Connected Health has a much broader definition. Essentially, to me, the term connected Health encompasses electronic health, wireless health, and mobile health and comprises any series of devices, services or interventions that are designed around the patient’s needs in such a way that the patient can receive care in the most proactive and efficient manner possible, preferably – but not necessarily, outside the acute care environment. The term implies that all stakeholders in the process are ‘connected’ by means of timely sharing and presentation of accurate and pertinent information regarding patient status. It involves building the system around the patient and connecting all of the stakeholders in the process. From my perspective it is a mistake to consider connected health in terms of the technologies that make it possible. Rather, we should consider it in terms of the connected, and therefore more efficient, services that it enables. It is also worth noting that Connected Healthcare can extend the reach of the healthcare system into the patient’s home, but is not confined to the home. Its optimal implementation implies that services and stakeholders are connected and coordinated across the spectrum from the home to the specialised acute care environment.

Clinician champions within each member hospital and lead clinicians for each disease will matrix to identify the best possible clinical input.Input from patients and patient groups through IPPOSI supporting structures and direct relationships with individual patient representative groups and online social networking platformsAn interdisciplinary network of engaged academics from UCD and partner universities. This process is supported by a central management function providing funding expertise and project management expertise. At its heart is a core research team that is both full time and multi-disciplinary.
