This document contains a monitoring tool used to evaluate a teacher, Melody Anne D. Evangelista, on her implementation of modular classes. The tool contains 6 criteria on distributing modules, monitoring student progress, providing feedback, connecting with students, interventions for struggling students, and participating in discussions. For each criterion there are options to rate the teacher's performance from missing several weeks of monitoring to exhibiting best practices in monitoring.
